What is Acne?
Acne is inflammation of the skin around hair follicles and sebaceous glands. Almost everyone suffers from this to a greater or lesser extent during puberty. Acne usually heals on its own around 25e year of life. In a small percentage, the disorder persists longer or only starts in adulthood. Doctors aren't sure why it's different for everyone.
It is partly hereditary. Acne is more common in some families.
Acne is not caused by insufficient urination, sweets or certain foods or drinks, makeup, smoking, stress, or sweating. Unfortunately, adjustments to these factors will not cure acne.
What causes acne?
Acne occurs when a hair follicle becomes blocked by sebum and becomes inflamed. Acne usually starts during puberty when the skin undergoes all kinds of changes due to hormones. Acne can also be caused by genetic factors, stress and cosmetics.

What is the best treatment for acne?
The best treatment depends on the type and severity of the acne:
- Mild acne (blackheads, small spots): tretinoin cream or adapalene (Differin)
- Moderate acne (inflamed spots): topical treatment combined with erythromycin or doxycycline
- Hormonal acne: tretinoin combined with other treatments
- Severe cystic acne: referral to a dermatologist
Tretinoin is often referred to as the gold standard due to its broad spectrum of action and proven effectiveness.
How long does it take for tretinoin to work?
Tretinoin works gradually:
- Weeks 1 to 4: adjustment phase, sometimes with temporary worsening of symptoms (“purge”)
- Weeks 4 to 8: fewer blackheads, smoother skin texture
- Weeks 8 to 12: visible reduction in spots
- Months 3 to 6: optimal results, smoother and firmer skin
What is the difference between tretinoin and Differin (adapalene)?
Both are retinoids and work in a similar way, but there are differences:
- Tretinoin: an older treatment, stronger, faster results, may cause more irritation
- Differin (adapalene): newer, milder, less irritation, suitable for sensitive skin
Tretinoin has a slightly broader range of effects (including anti-ageing), whilst Differin is primarily aimed at treating acne. The choice depends on your skin type and your goals.
How can you buy tretinoin cream legally in the Netherlands?
Tretinoin is available on prescription in the Netherlands. You can purchase it legally via:

Is tretinoin safe?
